Except in cases of emergency (see the Assistance and Priority Services page), to apply for a new passport to replace one that has been lost or stolen, it is nevertheless necessary to book an appointment through Prenot@Mi and provide the following documentation:
- Police report of the loss or theft of the passport;
- Passport application form (or Passport Application Form for Minors);
- Valid identity document, to be presented in the original and accompanied by a photocopy;
- Two 4 × 4 cm photographs of the applicant, taken frontally against a white background;
- Passport fee payment (to be made at the Consulate).
If you only wish to report the theft or loss of a passport (without applying for the issuance of a new passport), it is not necessary to book an appointment.
