- Passport Application Form (or the Passport Application Form for Minors), completed in full;
- Two good-quality photographs, 4 × 4 cm, with a white background (applicants are advised to check in advance that the photographs comply with the required standards);
- Expired or expiring Italian passport (except in the case of a first passport application);
- Passport fee payment (to be made directly at the Consular Office).
When applying for a passport for a minor, the consent of both parents is required.
