Required documents for the issuance of the CIE

The applicant must bring the following documents:

1. Application form, duly completed in all its sections (for adults, click here; for minors, click here).
2. A valid identity document of the applicant and of all persons required to sign the application (original and photocopy of both sides).
3. The applicant’s valid Italian passport, if held, and a photocopy of the page showing the photograph.
4. The applicant’s previous Italian Identity Card, if held, and a photocopy of both sides.
5. One passport-size photograph, 4 × 4 cm or 3.5 × 4.5 cm, with a white background and good image quality. Please note: applicants are advised to ensure that the photographs meet the required standards. Photographs that do not comply with the requirements cannot be accepted.
6. Consent from both parents if the applicant is under 18 years of age.
7. A declaration of loss or theft of the previous identity card, if applicable.
